Which incidents must be reported to the Department of Employment and Labour?
Serious incidents — including fatalities, injuries likely to cause death or major impairment, and certain dangerous occurrences — must be reported under the OHS Act. Internal recording, though, should cover every incident and near miss; the pattern is where prevention lives.
What is COIDA and why does this form matter to it?
COIDA is the Compensation for Occupational Injuries and Diseases Act — the system that compensates workers injured on duty. The employer must report workplace accidents for the claim to proceed; your incident report is the factual basis for that submission, and delays or gaps in it directly delay the worker's compensation.
Should near misses really be written up?
